Independent insurance agents can register to submit quotes and manage their SFM book of business through SFM Agency Manager (SAM). Injured workers who have workers' compensation claims with SFM can access claim details online through the Claim Connection. Providers who treat injured workers covered by SFM can access bill status, EOBs and claim information through the Provider Connection. As soon as you learn that one of your employees has been injured, report the injury to SFM using one of the methods below. Please note that if it's an emergency, call 911 or get the employee to an emergency care facility before reporting the injury to SFM. If you are part of the Wisconsin Worker's Compensation Insurance Pool, please visit the reporting work injuries as a state program employer page.
You are eligible to use this service if you are invoiced directly through SFM instead of through your agent. You can make a one-time payment, or register for an account to set up AutoPay and see past payments or invoices. Premiums are billed at the beginning of the policy period, based on an estimate calculated from your organization's job classifications and expected payroll. Classifications describe the nature of your business and reflect the risk of injury. Payroll can change throughout the year, due to changes in workforce size, employees working overtime or operations expanding or diversifying. At the end of each period, we audit the past policy period and classifications and adjust your premium to reflect any change, if necessary.

Don't answer phone calls and voicemails, haven't replied to very many emails. Seek a company that actually values it's customers. Not very happy with this company at all.

Critical: Quality, Responsiveness Very shady. Unprofessional. Don't pay on their claims. Michele Paton is a vengeful individual. This woman will do anything to make the injured employees look bad! THIS COMPANY AND MICHELE PATON ARE UNSYMPATHETIC, MONEY HUNGRY CROOKS.

They made me go and report to their doctor so he could provide his opinion as to what caused my injury. Big shocker it wasn't work when that's what caused it. But the concerning part was they made me go to a Doctor Who refused to wear a mask because he claimed he had antibodies in his system, therefore he could not transmit it to anyone. This was when Covid started coming out.

And now to this day we still don't even know if you can have antibodies and transfer it or not to someone. So 100% they sent me to a Doctor Who very likely could've given me Covid and refused to take any precautions for it. Super professional, I say with the most sarcasm possible.

They use shaddy tactics and to get what they want, I believe if someone investigated them they would find all sorts of illegal activity. Not only do they despise minorities but they fight tooth and nail to discredit us and put us down paying whoever they can to do it. You will pay for this!

Critical: Professionalism Beware of personal information, I asked the company if they do backround checks on employees or any contractors and they would not reveal that information. Adding insult to injury they were surveiling my whole family including my 4 and 2 year old kids. And they dont have enough decency to do backround checks on people.

My claim handlerfrom sfm in South Dakota is verbally abusive. I have to hire an attorney to talk to them, withheld checks causing me to lose security deposit and extra charge for overstay at motel room I was in. denied surgery to injury.

SFM is a very difficult company to deal with if you have a workers comp claim. They make it a fine art on how to delay your claim and to not pay it. Be sure to be represented by an attorney for a serious claim or you will leave money that you deserve in the SFM's bank account. All of you should know that your attorneys fees are paid for by the worker's comp fund.

We are looking elsewhere for next years renewal. Audios SFM Update, it has gone from bad to worse after five months. This company cut off benefits and it will take a judge to reinstate. Final update. The claim finally finished and probably the most difficult negotiations in my life. I assisted a dentist with her claim for carpel tunnel in both wrists.

After two years she is still totally disabled but the insurance company expected her back to work after a few weeks. The emails were constant and once we decided to hire an attorney it went from bad to worse. The company cut off all workers comp payments until we settled the case. Read the bad reviews they have and judge for yourself.

Terrible Company! I went through a Workers Comp audit in Aug this year, turned in all the documentation requested and they randomly cancelled my policy? No notification just sent cancellation letters to our builders. I call to figure this out and was told my auditor was missing information requested. which she wasn't because I produced the emails of all documentation dated Sept 7th 2018!

The auditor will not return my calls, calling into to this company is no help and people are rude. I've been with this company for 8 years and this is what I get? Mind you this is the 2nd time they've screwed up my audit! Beware poor customer service! I'm on day 4 with no workers comp thanks to this company and still no resolve.
