A traditional "gatekeeper" model Health Maintenance Organization (HMO) plan requires members to select a Primary Care Physician. Referrals to participating specialists, fully-integrated medical management, and comprehensive benefits are subject to small co-payments. There are no out-of-network benefits.
These plans are custom tailored by the employer-sponsors and Reliable handles the administration. They may vary from the standard plan designs and co payments however, they follow the general rules of each product type as described above. Offers self-insured plans a defined benefit structure based on more economical reimbursement levels designed to be fair and reasonable to providers based on various pricing data sets, most notably Medicare.
